Why it works

Azelaic acid is antibacterial (including against acne bacteria), anti-inflammatory, normalizes the keratinization that clogs pores, and inhibits tyrosinase in overactive pigment cells — selectively targeting hyperpigmentation while sparing normal skin.

Concentration matters

Prescription strengths are 15–20%; cosmetic products typically 10%. Most trial evidence is at the higher strengths — expect slower results from OTC concentrations.

A 2023 systematic review counted 43 randomized trials of azelaic acid. For acne, 15–20% formulations beat vehicle and, in head-to-head trials, performed comparably to benzoyl peroxide, erythromycin, and tretinoin — expect temporary tingling or itching at first. Most trials used prescription-strength versions.
